How to set Discord, Owner and Teamspeak in Server List

Hey, i just saw a Server which set a Owner, Discord and a Teamspeak3 in the Server List Now im Asking how can i add a Discord Server, Owner or Teamspeak Server to be shown like this: http://prntscr.com/qgu0e1
