Can't respawn after dying, infinite black loading screen

On my clean server, no mods installed everytime you die, you get stuck in the infinite black loading screen. Is there something I’m missing? Here’s the video: https://www.youtube.com/watch?v=i56Afisjyso

This happened to my friend and I, in the LAMBDA menu, go to the Settings Menu.
On the second page of the Settings menu there should be an option called “Restore Appearance On Respawn”.

Make sure this is set to OFF.
