You’re probably not executing the permissions.cfg or your server is outdated.
There’s no real reason to provide updates right now. There’s no new features that I’m working on (too busy with other projects) and there’s no critical bugs that I can fix. There is a bug that crashes the server on Linux when using SQLite, but so far I’ve not figured out at all what’s causing it. I can’t reproduce the crashes myself, since it always works fine for me no matter what.
You can’t disable all features in the misc settings menu. Only some options can be disabled in there. Things like save preferences and other harmless features are always available.

I need more info than that, that’s a super random issue and nobody has ever had this same issue. So I’d need a bug report with exact steps on how to reproduce this on a fresh server, with the latest version of vMenu installed.

There’s multiple resources that do this. Or even client side mods if you have scripthook allowed. For vMenu there’s permissions, if someone doesn’t have permission to use the option, then there will be no player overhead names created by vMenu. Disable vMenu, and check if the overhead names are still there. If they are, then it’s not created by vMenu. If they’re gone, then it’s created by vMenu and you should disable the permissions for it for anyone that you don’t want to have overhead names.

Download it from github, follow the instructions on the docs. It’s really not that hard, the __resource.lua is included in the download, if it’s not, then you’re downloading the wrong files.
