So when you go to some where and die and then go towards some where else and die then get revived you teleport to the last location.

Which script would this be?
Do you have a revive or respawn script ingame? If so that is likely bugging out and reviving you even though you not dead.
I have removed all fadein / fadeout effect and it worked fine
