Why whenever i die i cant open the menyoo again forcing me to relog.

so for some reason when i die it wont let the menyoo open
People are Very Confused on what a Server Actually is though everything I have found is Client Sided but they Say Server
It keeps crashing me whenever I open Player Options and it shows this. It is kinda important for me because it breaks the death script, I’ve tested it without and the death script works so it is for sure this that is causing the error.

i have all these stuff, it used to work great till when i got the new dlc build thing. now for some reason i cant make this menu work.
tried using older server builds but still isnt working. i would appreciate it if anyone could help me with this. btw the server is mine and i have enabled script hook which is sv_scriptHookAllowed 1.
