You haven’t even said what the issue is. Shouldn’t the Collective be first point of contact if there is an issue, so they can rectify it? What’s wrong with the format specified?
Good point Shayan, I posted the problem in another place, My bad there! it was the bug report below this one.
Moral of this entire post was , I normaly get better answers from the community. Just found it a joke I need to fill out the form or it may get kicked, There is allot of that. If they don’t like it they remove it and so on. Rather get them drunk on Vodka first then fill out the form.

The form is there for a reason, if you cant be bothered to fill it out correctly its no one elses fault but your own.
We have never penalized someone for not filling out the format
